Assessment Application Requirements
Applicants may be referred by a medical practitioner, hospital, clinic, or may contact St Giles directly to request an assessment and rehabilitation services.
Please provide a copy of the patient’s National ID, Birth Certificate, or Passport for identification and record-keeping purposes.
Submit any relevant medical reports, test results, discharge summaries, and medical aid or insurance information to assist with assessment and treatment planning.

Provide details of any previous treatments, therapies, surgeries, rehabilitation programmes, or specialist consultations related to the patient’s condition.
Please provide a list of all medications currently being taken, including dosage and frequency, to ensure safe and effective care planning
